Complete your Canadian T4A tax form
Complete the Canadian T4A tax form when Dots asks you to provide tax information as a Canadian payee. You first choose between the resident and non-resident forms, then fill in the fields for your entity type and consent to electronic delivery.
Choose your T4A form
On the Canadian Tax Form page, choose which form applies to you:
Fill form T4A (Canadian Resident) — for Canadian residents.
Fill form T4A-NR (Non-Resident) — for non-residents.
Fill out the T4A (Canadian Resident) form
The T4A form asks for your entity type, your address (Address Line 1, City, Province, Postal Code, Country), and your consent to electronic delivery. The country defaults to Canada and can't be changed.
The remaining fields depend on your entity type:
Business: Business Name and Business Number (BN). The BN must be nine digits.
Individual: First Name, Last Name, Social Insurance Number (SIN), and Date of Birth. The SIN must be nine digits, and the date uses the format YYYY-MM-DD.
Fill out the T4A-NR (Non-Resident) form
The T4A-NR form asks for your address (Address Line 1, Address Line 2, City, Province/State, Postal Code, Country), your country of residence, and your consent to electronic delivery. The country stays selectable for this form.
The remaining fields depend on your entity type:
Business: Business Name and Business Number, if supplied.
Individual: Legal First Name, Legal Last Name, Social Insurance Number (optional), Foreign Tax ID Number, and Date of Birth.
For individuals, the Foreign Tax ID Number is required and must be alphanumeric, and the Country of Residence is required.
Consent and submit
Both T4A forms require you to check I consent to electronic delivery before you can submit.
Submit the form to finish. When you complete it outside a flow, you'll see the message Form submitted successfully!
